- Formerly owned by Pura, daughter of Nehemiah of Modena (f. 1r; Javier del Barco).
- Inscription documenting the sale by Binyamin Desforno, son of Rabbi Shem Tov Desforno, to Rabbi Isaac, son of Rabbi Uziel of Modena, 22 January 264 (1504), witnessed by Abraham Yehudah, son of Rabbi Yehiel, of Camerino and another witness (f. 396r). A transcription and translation by Javier del Barco is in the curatorial file.
- Offered for sale by Bernard Quaritch (London), catalog 176 (1898), number 6; catalog 290 (1901), number 31; and catalog 341 (1916), number 218.
- Sold by Wilfrid M. Voynich to the Yarnall Library of Theology, St. Clement's Church (Philadelphia, Pa.), October 1917, Yarnall accession number 1377. The Yarnall accession entry includes the note "Rare MS written in Italy before 1480."
