Paper, i (contemporary paper) + 199 + i (contemporary paper); [viii], 1-390; contemporary pagination in ink, upper outer corners. The fourth folio appears to be a later addition.

Contemporary parchment. Boards badly warped, worn at edges and cracking at hinges and spine. Indications of possible worm damage on spine. Fourth folio very loose. A few light stains and smudges.

Written in a cursive script by a single hand.
