- Written for Giovanni-Maria Viscount of Sforza, Duke of Milan ?; Trivulzio-Belgioioso-Trotti Collection, Milan, sold (1885) to Hoepli; sale by Leavitt, New York, (1888); Thomas Richardson, Boston; John Frederick Lewis, Philadelphia (obtained 1919); given by his widow, Anne Baker Lewis, to the Free Library of Philadelphia in 1936; notes written in pencil by Lewis on inside of front cover; paper with the signature of T. F. Richardson and "no. 17"; on pasted on to inside front cover; tipped onto the front flyleaf is a hand-ruled page of notes written in Italian in an eighteenth-century hand that describes the manuscript as being made for Giovanni Maria Visconti of Somma; signature and stamp of Lewis on the recto of the first flyleaf, dated 1919; stamp of Richardson on recto of front flyleaf; cutting from a manuscript pasted onto the recto of fol. 78, probably from a prayer book; on fol. 1r the coat of arms of the Sforza family
