The added prefatory leaves are foliated in modern pencil: "a" (modern(?) flyleaf), i–xxiv; the main text is foliated in medieval ink arabic numerals, presumably by the same person that added the alphabetical index: 2–256, accidentally omitting "88", and missing the following due to the loss of leaves: 1, 30–31, 122, 135, 148, 172–174, 201, 203–206, 218–219, 239, 241, and 243; followed by one old and one modern(?) flyleaf, foliated 257–258 in pencil; many of the pages foliated in medieval ink arabic numerals also have supplementary modern pencil foliation, we follow the modern pencil foliationCollation

Late 15th- or early 16th-century Oxford blind-stamped binding. Tanis reports a "crescent stamp", which he compared to Weale, Bookbindings (1894), p.104 no.48 (not illustrated), but I see no such stamp: the front board (very worn) has what appears to be a stag's head or skull within a rectangle c.11×7mm, within an oval frame c.23×16mm, and the back board has two columns of six stamps of a double-headed eagle with splayed wings, within a square c.17×17mm, surrounded by a border of rampant lions, within a square c.14×14mm; the edges of the leaves are inscribed/decorated: Tanis saw the initials "B G", but it is more likely that the edges are decorated with foliate scrolls, especially as the fore-edge has three letters/scrolls; repaired and re-backed by Harold Tribolet, of Chicago, between 1964 and 1975 (see Holloway, p.68); stored with the manuscript is an envelope marked "Remnants of back board" containing four pieces of worm-eaten wood with holes and channels for the sewingLayout

- Written in England, doubtless Oxford, and completed (just as the Black Death was arriving in England) on the day after the feast of Corpus Christi (i.e. Friday, June 20), 1348: "Fuit scriptus iste liber Anno incarnacionis domini M o .CCC o .xl o .viii o . Die veneris proxima post festum corporis Christi", and in red: "Completo libro sit laus et gloria Christo. / Explicit lilium medicine." (fol.256v); John Cokkes (d.1475), Oxford physician: inscribed: "Istu(m) libru(m) emi a doctore Kokk(es) p(ro) x s et adhuc dedit m(ihi) i(n) p(re)cio / xl d pro div(er)s(is) laboribus in medici(nis) et precium xiij s iiij d .", 15th century (fol. 2r, top margin), doubtless referring to the John Cokkes (Cokkys / Kokkes, etc.), who apparently wrote Oxford, Bodleian Library, MS. e Mus. 155 (and signed it in several places, including, at p.507, "correctus et scriptus per Johannem Cokkes manibus suis propriis Oxon."), owned Bodleian, MS. Ashmole 1432, and wrote or composed at least part of Cambridge, King's College, MS. 16 (fol.8r includes "Quia ego Iohannes Cokkis …"); see A.B. Emden, Biographical Register of the University of Oxford to A.D. 1500, I (Oxford, 1957), p. 457, which refers to a document in which Master John Cokkes, physician, is accused of withholding evidence relating to a patient's death; A thoroughly-effaced seven-line inscription in 16th-century italic script in which the first appears to begin "Audivi …" and the last line "Ex …" (fol.ir); below this, perhaps written by the same hand, is "Gallicina' hunc Bern: de Gordone appellatur"; inscribed, perhaps by the same person in less formal script, with a note beginning "vide fol.38: De antidoto Lepræ, a serpentibus …" including "Gallicinia vipeas appellat" (fol.i verso); Inscribed "W. Latten" (fol.1r), 17th(?)century, perhaps one of the three Oxford men called William Latten/Latton recorded by Joseph Foster, Alumni Oxonienses: The Members of the University of Oxford, 1500–1714 (Oxford, 1891), pp.884–85; the name was read as "W. Patten" by de Ricci and Tanis (a name that does not appear in Foster), before the name is a price(?), apparently "0.2.6", perhaps later altered to "1.12.6", and after it a number or date, apparently "1555", but apparently not in the same ink as the name; Inscribed with a 17th(?)century shelfmark(?) ".E. / .9."; Inscribed with 17th/18th(?)century initials "AR" in monogram (fol.137r, lower margin); Sir William Osler (1849–1919), Canadian physician, of Montreal and Oxford (on whom see the ODNB and Harvey Cushing, The Life of Sir William Osler (2 vols, Oxford, 1925, reprinted New York, 1993), acquired probably in Oxford in 1915 (see ibid., II, p.506, quoting a letter written on 10 Dec. 1915: "I am picking up a few treasures—nice XIII Cent. MS of Platearius—one of the Practica of Bernard de Gordonio."), with his bookplate printed in capitals: "From / the library / of / Sir William Osler, Bart. / Oxford" (front pastedown), inscribed "7523"; inscribed in pencil with the same number (fol.i verso); shortly before his death he made lists of books which should be given to particular institutions, including to the "College of Physicians Phila –that Mon[t]pellier MS. Textbook 1373 [sic]. W.O." (see Cushing, op. cit., II, p.683; cf. Bibliotheca Osleriana, 1929, p.xxxi); bequeathed in accordance with his wishes to: The College of Physicians: the manuscript was not actually transferred to the College until December 1928 (almost a decade after Osler's death), presumably when the Osleriana catalogue had finally been sent to the press (Elizabeth M. Holloway, "The Oslers and the College of Physicians of Philadelphia: Some Further Footnotes to a TwiceTold Tale", Transactions and Studies of the College of Physicians of Philadelphia, 4th series, 43, no. 2 (October, 1975), pp.64–8, at 67–8); the Library bookplate (back pastedown) is dated "Dec 24 1928" and has an accession number(?) "127743" and valuation(?) "200. 00 "
