Red leather over pasteboard with simple blind-tooled frame of double lines with three lines intersecting in the middle; blind-stamped central mandorla and eight pendants, two on each of the vertical and horizontal axes.

18 lines in one column along inside margin with another column of notes bordered in red.

Comments border ruled in red; rubrications in red; two small illustrations (f. 10v, 86v).
